Submit a Claim - Claim Instructions

If you are a person who, as the Original Owner or Lessee, purchased or leased for personal, family, or household use, a new 2023 or 2024 Ford Transit Trail from an authorized Ford dealer before June 1, 2024 (“Class Vehicle”), you may submit a Claim to receive a $6,000 payment. You will need to provide supporting documents proving that you purchased or leased the vehicle, for example a Certificate of Title, current Vehicle Registration, DMV records, or current proof of insurance.

If the Class Vehicle has more than one owner or Lessee, only one Claim Form should be submitted, and any settlement payment will be issued as a single payment in the name of the claimant.

Your claim must be submitted by January 15, 2027. If you submit it by mail, it must be postmarked by January 15, 2027.


How to File Online

Before Claim Filing: You will need the Unique ID printed on the Postcard Notice that you received in the mail. If you did not receive a Postcard Notice or have lost yours, please contact the Settlement Administrator at 1-877-525-1779 for assistance.

During Claim Filing: As part of filling out your Claim Form, you will be asked to verify your contact information as well as details about the vehicle you own(ed) or lease(ed) including the date of purchase or initial lease. You must also provide documentation showing that you were the Original Owner or Lessee. Finally, you will need to provide tax information so that if your Claim is approved, we can issue you the appropriate IRS Form 1099. You must provide proof that you were the Original Owner or Lessee of the class vehicle. Acceptable supporting documentation includes:

  • a copy of the original purchase or lease agreement reflecting the name of the seller/lessor, the purchaser/lessee, the VIN, and the date of purchase/lease; or,
  • other reliable documentary proof sufficient to show your identity, the VIN, and the date of the first retail purchase or lease, including but not limited to a buyer’s order, retail installment sale contract, lease summary, bill of sale, certificate of title, initial vehicle registration, temporary registration, DMV record, financing or leasing record, dealer or intermediary invoice, or other comparable documentation.

After Claim Filing: After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and confirmation code and refer back to them if you have any questions about your Claim Form.

How to File by Mail

If you wish to submit Claim Form via standard mail, you may download a copy of the Claim Form here. You will need to provide all the information requested on the Claim Form, attach any supporting documentation, sign it, date it, and then mail it to the following address:
